What Are Net Carbs?

I have type 1 diabetes, and my son recently gave me a package of sugar-free hard candy that was labeled "0 net carbs." The back of the package said, "To calculate net carbs, subtract the sugar alcohols from the total carbs in the product, because sugar alcohols have minimal impact on blood sugar." I am concerned and confused about this labeling.

Wheeler, MS, RD, CDE, FADA, CD, responds: The term "net carbs" came about when the "low-carb" food fad began a decade ago and companies were seeking a way to market their products as being low in carbohydrates. A food's total carbohydrate count (in grams) is arrived at by subtracting the grams of protein, fat, moisture, and ash from the food's total weight. What's left is the total carbs. On a nutrition label, the total carb count is required by the Food and Drug Administration (FDA) to include the full amount of grams from sugar alcohols and fiber. However, these carbs have less impact on blood glucose than others because they are only partially converted to glucose, or not at all, by the body. Some food companies started using the term "net carbs" and defined it to mean the total grams of carbohydrate minus the grams of sugar alcohols, fiber, and glycerine. This equation is not entirely accurate, because some of the sugar alcohols and fiber are absorbed by the body. In fact, about half of the grams of sugar alcohols are metabolized to glucose.

The term "net carbs" does not have a legal definition, and it's not used by the FDA or the American Diabetes Association. When you see it on a label, you should read the nutrition facts and ingredients list for more information. If you are not on intensive insulin management and do not count carbs, you don't need to do anything. If you are on intensive insulin management, count carbs, and manage your diabetes with carb-to-insulin ratios, you can:

1. Check the product's sugar alcohol content. If it is greater than 5 grams, subtract half the grams of sugar alcohols from the total carbohydrates and count this as the "available carbohydrate" for insulin adjustment purposes. However, if erythritol is the only sugar alcohol listed, subtract all of the grams of sugar alcohol.

2. Check the fiber content. Total fiber in foods comprises many different types of natural fiber and manufactured ingredients. They may vary in whether they are digested and how they affect blood glucose. If insoluble fiber is listed on the nutrition facts panel under "Total Carbohydrate," subtract all of the insoluble fiber from the total carbohydrates and from the total fiber grams. If the fiber quantity is still greater than 5 grams, subtract half the grams of dietary fiber from the total carbohydrates, and use the result as
the available carbs for insulin adjustment purposes.

new to insulin management,

I am trying to count carbs and can find lots of interchanges for different carbohydrates but nothing that says "have x number of carbs per day. ? A 1500 calories a day requires ? carbohydrates etc.

carb for a 1500 day/200 grams

the total number of carbs you can have if you are not dieting and want to only consume 1500 calories a day is 200 grams.
however ifyou are on a carb sensitive diet and are in the induction period it is only 20 grams for 2 weeks, 25 - 50 for the rest of the weeks until you reach your goal ( not exceeding 1500 calories a day ) and then after you achieve you goal you can adjust the number of carbs slowly to what your body can handle without gaining weight. there are some foods you just are not going to get to have any more if you want to keep the weight off. its a lifestyle moreso than a diet. goodluck! it works ive lost 50 lbs so far and have 50 more to go. however i set my sites on a realistic goal. you shouldnt lose more than 2 - 2.5 pounds per week. it will reek havoc on your body.be aware that there are false hoods out there that tell you that net carbs are just as enjoyable. i would forget that all together. at least in the first two weeks. it has not been proven that you can subtract fiber and sugar alcohols from the carbs to get a 'net carb'. a carb is a carb is a carb. please read the diabetes.org section about sugar alcohols and fiber. they do not approve this counting measure.

Based on what you say above regarding sugar alcohols other than erithritol, I had a question: you suggest that if the sugar alcohol content is higher than 5 grams, you should subtract half from the carb count, but what if it is between 1 and 5 grams (as many low-carb desserts and protein bars are, I'm finding)? Can you subtract 100%, rather than 50%, of the sugar alcohols grams if they are 5 or less?

MY HUSBAND AND I HAVE BEEN TYPE 2 DIABETICS FOR OVER 10 YEARS,I HAVE BEEN ABLE TO CONTROL MY LEVELS BY GOOD DIET. HOWEVER MY HUSBAND IN 2008 HAD LAPBAND SURGERY, TO LOSE WT, 138 LBS, TO DATE, BUT RECENTLY, HIS BLOOD SUGAR WAS UP TO 300 AND A1C 7.2.: HE FIRST STARTED KEEPING A WATCH ON SUGAR NO MORE THAN 5G PER SERVING, BUT THE DR GAVE HIM A CARB WATCH PAMPHLET. HE HAS BEEN TRYING TO EAT 6 SMALL MEALS A DAY, AS HE IS LIMITED TO HOW MUCH HE CAN EAT THRU HIS PROCEEDURE, HE IS A ROAD SUPVR AND IS IN AND OUT OF HIS CAR 10 HRS FOR 4 DAYS A WEEK. SOMETIMES HE FINDS IT HARD TO GET A BIT IN, EATING A HANDFUL OF ALMONDS IF HE CAN'T EAT WHAT HE MADE, TRYING TO STAY UNDER 25 CARBS PER MEAL. DOES ANYONE HAVE SOME SUGGESTIONS. HE TAKES COTTAGE CHEESE, 2 SLICES OF SWISS OR PROVOLONE, AND HAM, TURKEY OR CHICKEN SLICES IN A ROLL. HE DOES NOT LIKE VEGETABLES, BUT

Low carb lunches

If he likes BLT's I have used lettuce leaves sliced tomatoes, bacon and real mayo..........by using one leaf of lettuce and loading up on everything else he should do ok...especially if the lettuce is really fresh. Good luck low carb smoothies are also good and loaded with protein.
