How to Treat Diabetic Diarrhea?

I have a problem that I never see addressed. I’ve had type 1 diabetes for 36 years and been diagnosed as having diabetic diarrhea. Numerous tests have ruled out all other gastrointestinal problems. Is there any treatment for this problem?

Janis McWilliams, RN, MSN, CDE, BC-ADM, responds: People with diabetes may, of course, develop diarrhea for a variety of reasons, just like anyone else. Diarrhea is a symptom of many diseases such as viral or bacterial infections, celiac disease, irritable bowel syndrome, and Crohn’s disease. It is a side effect of some medications, such as metformin, and some sugar-free sweeteners can cause diarrhea in some people.

However, diarrhea can also be a symptom of a type of autonomic neuropathy. This is what is known as diabetic diarrhea. Although this condition is not uncommon, the diagnosis is usually made only after a detailed history and diagnostic tests reveal no other cause for the diarrhea. 

Unlike the more widely known peripheral sensory neuropathy, which affects the hands and feet, autonomic neuropathy damages the nerves that control involuntary activities of the body. More commonly known types of autonomic neuropathy include erectile dysfunction and orthostatic (or postural) hypotension, the feeling of light-headedness or dizziness you get from standing after lying or sitting down.

Diabetic diarrhea occurs usually at night, is watery and painless, and can be associated with fecal incontinence. Bouts of diarrhea can be episodic, along with intermittently normal bowel habits or even alternating with periods of constipation.

The treatment for diabetic diarrhea is individualized, but it generally starts with antidiarrheal agents such as Lomotil (a combination of diphenoxylate and atropine) or Imodium (loperamide). High-fiber foods or bulk-forming laxatives such as Metamucil may help decrease the symptoms. As with all neuropathies, good glucose control is important in controlling the symptoms.

Your doctor may order antispasmodic medicines to decrease the frequency of bowel movements. If bacterial overgrowth in the intestines is thought to be present, antibiotics may be ordered. Medications like clonidine or octreotide, which have other primary uses but have been shown to help diarrhea, can be used in more advanced cases that do not respond to other treatments. Although your primary care doctor or endocrinologist can initiate treatment for diabetic diarrhea, a referral to a gastroenterologist may be indicated when standard therapies are ineffective.

Diabetic Diarrhea

My mother indeed has Diabetic Diarrhea but it does not occur only at night but also all day long. Sometimes she stays with diarrhea for two straiht months and we don't know what else to do. She lives in Brazil would you have any reccomendations?

I too think I have Diabetic Diarrhea. I have been to many doctors, but have found no relief. It is ruining my life. If I eat anything, I get the horrible diarrhea. I have horrible pains in my abdomen, and take lomotil by the bottle. They have diagnosed me as having microscopic colitis, but I believe that it is Diabetic Diarrhea. Nobody wants to consider that this is what it could be. I have been a diabetic for 39 years, and have not had good control. If you are able to find someone to help your mom, please pass on what you find out. I wish I could be more helpful.

I too have been diagnosed

I too have been diagnosed with microscopic colitis. I have not been diagnosed as having Diabetes, but it runs in my family.

I get light headed/ dizzy, headaches, blurred vision, and I can not eat anything without running to the bathroom within 5 minutes from eating it.

I have had every stomach test they can think of.

I'm not sure what to do next.

Its not fair that my hubby has diabetes because he is NOT overweight. There is a lot of fat people who don't care what they eat and they are the ones that should have this, not someone who is not fat and never ate junk! I have an overweight (230lbs) and says "I know I am gonna get diabetes my mother had it" but then she continues to gorge herself with junk food and crap, makes me so mad I want to say "you are the one that should have diabetes NOT my husband". But I am to nice to do that. Life is not fair and of course...no one said it would be.

Just because someone is over weight does not give you the right to bash them. Have you walked in there shoes? No you have not. There can be reasons why they can't get out and exersice. My doctoe will only let me exercise for 5 minutes. I can't get outside much because of some of my meds. My mother lives with me and when we go out to eat we split a meal. Not because of the price but because of the portions. If we go to a buffet we get 1 meat and good vegetables and only get 1 plate and a salad. I don't think you are that nice or you would have never wrote this in the first place. I don't go around eating junk food all the time, we don't bring into our home. So just because someone you know does that don;t mean everyone does.

I'm 18. 5'11 and 175 pounds and I am a diabetic of 3 years. You don't see me bitching and wishing it upon anybody else. That's just rude. I would never put this disease on anyone like that.
